Prescription Drug Errors: A Growing Problem

Prescription errors injure at least 1.5 million people every year, according to a report from the Institute of Medicine of the National Academies. Most of the errors are preventable mistakes that our health care system failed to prevent.

Most injuries and deaths could be prevented if physicians, nurses, hospitals and pharmacists are careful and make use of available technology to prevent errors, many injuries and deaths. The most common types of medication errors include:

· Administering the wrong dose of a drug. The right dose of a drug can heal, but too much may be fatal.
